Nurse's Notes

Things to know about the Nurse's Office:
****If your child has been home sick with a fever or vomiting, please keep them at home for 24 hours after the fever or vomiting resolves.
***A parent, guardian or a parent /guardian designated, responsible adult shall deliver all medication to be administered by school personnel to the school nurse or other responsible person designated by the school nurse. If your child requires medication, click here to print the required forms
***Students are not permitted to carry any medication, except Inhalers or Epi-pens.
***Please use the absentee line to report your child’s absence.
***Only one physical is required upon entry to first grade. The preschool/kindergarten should have an annual physical due to their age.
***All immunizations, sent to the school, should have a signature from the practitioner on them. If your child receives updated immunizations, please provide them to the nurse for their health records.
***If your child has an injury requiring crutches or immobilization of an arm or leg, please call the nurse and I will provide assistance to the child. Any absences from PE and gym should have written documentation from your child's medical provider.
